Our Support
Collaborative Care Services provides CQC regulated care and support services fo cater for individuals with complex needs. We focus on supporting young people and adults to live a better quality of life in their homes and communities. We pride ourselves in a fully trained team of professionals who are compassionate and committed to empowering the people accustomed to our care to live meaningful lives with an increased level of independence.
Who We Serve
We support and motivate children 0-18 years and adults under 65 years and over to be independent and be integrated into the community. We work with:
- Children 0-18 years and adults under 65 years and over.
- Support with Domiciliary care and Supported living services
- Those who live with Learning Disability and/or Autism Spectrum Disorder
- Mental Health
- People with Eating Disorder
- People who misuse Drugs and Alcohol
- People with Sensory Impairment
- Those who have family or relationship breakdown
- Those with complex or challenging needs
- Those going through emotional difficulties
Our Objective
Collaborative Care Services will assess the needs of young people and adults through the use of assessment plans. To achieve our purposes we aim to assist our clients in producing the following results in their lives:
- Healthy and satisfying relationships with their families, friends and peers
- Health service that is tailored according to the needs of each individual.

- Sustainable employment, training and education
- Financial and tenancy management
